Pothole in northbound lane

Tree looks ready to fall onto Mt. Vernon. South side of the road. Appx 100 feet east of Corners Drive

Two potholes on the westbound side of Mt. Vernon. Just east of Northchester Ct.

Street light on SE corner of Jett Ferry & Dwy Club Drive. Both lights are cycling off & on throughout the night

Bocce ball court closest to the interior of the park is floodedand not draining water.

Center line "turtles" along Mt. Vernon in front of Nandia Lane are missing.

Wrong way drain grates on Stormwater drain in roadway. This appears to have been installed in recent years. Would like to know in our design standard for installing drain grates that we install them perpendicular to the direction of travel. This is very dangerous for persons riding on bicycle.

Change the to drain grates on the side road coming off of DeKalb Avenue into the parking lot that is adjacent to the amphitheater. The current ones have elongated grates which are dangerous for bicyclists. Replace with smaller hashed (etc.) grates.

Please clean the bike lanes on Dunwoody Village Parkway. There are several places with gravel and other road debris along both directions. Thanks!

Missing Bike Lane Begins / yield Right Turn to bikes / Bike Lane End signs on Peachford. This is near the intersection with North Peachtree. Bike Lanes were created in 2013 and new signage per MUTCD needs to be installed.

The bike lanes do not have markings. Please use thermoplastic markings, or use the same markings that were used on Perimeter Center East last year.

Glass & accident debris in bike lane / paved buffer. Westbound lane on Mt. Vernon, at the merge with Ashford Dunwoody. Has been here over 2 weeks.

In the new bike lane on the south side of Mt. Vernon (eastbound direction), there is a pothole a several inches deep and in width (appears as if the top layer of new asphalt has simply "broken away"). It is located approximately 200 - 400 feet west of the Ashford-Dunwoody intersection.

In the new bike lanes on Mt. Vernon, there is a good amount of gravel, ground asphalt, debris remaining. Making it unsafe to ride a bicycle in the lane.
